According to The Star, it looks like a good portion of the blame can be attributed to Cliff Fletcher. Damn guy managed to kill the leafs more when he was interim GM, and he's doing the same to Pat now

This isn't like the first rumour about Burns dead? Who really wants him dead so bad that s/he has to do it twice?
