That fire was caused by sloppy engineering, no doubt about that but unless you put a fuse inside the battery there is no way to protect against a dead short. The incident was across two 12v batteries connected in series inside the pack so it only took 24v to start the fire. It probably wasn't enough...

Yes, if you build the driver like a traditional headphone driver with who knows how many ohms impedance. Chop down the impedance and it would work.
