Have you tried running FC5 on Very High or High and see if you're still capped?

You don't have a frame rate cap enabled in the game, do you? Also, you're not using DSR are you?

If FC5 is acting up, it might be easier to reinstall FC5 than to reinstall windows, so that's worth a try.

If those clocks are sustained through the runs, the GPU is fine. The CPU utilisation under load is looking too low, probably. Your LCD is far more likely to have a few pixels not working than your GPU. Have you tried your video output from your motherboard on your screen to see if dead pixels are still there?
